Audit Trail
Every action taken on a HazCom plan is recorded in a dedicated audit trail. This is not just a convenience feature -- it is a critical compliance tool. When OSHA inspectors ask "When was this plan last reviewed?" or "Who approved this version?", the audit trail gives you documented proof.
For: Admin, Coordinator, Manager | Tier: Standard+
What Gets Tracked
Tellus logs 16 distinct action types organized into 5 categories:
| Category | Actions Tracked |
|---|---|
| General | Plan created, Plan updated, Plan deleted, Plan viewed |
| Section | Section answers updated, Section content edited |
| Workflow | Submitted for approval, Approved, Rejected, Published |
| Version | Version created, Version archived, Version restored |
| Export | PDF exported, DOCX exported, Preview generated |
What Each Entry Contains
Every audit log entry records:
| Field | Description |
|---|---|
| Timestamp | Exact date and time of the action |
| User | Who performed the action (name displayed) |
| Action | What was done (one of the 16 types above) |
| Action Category | The grouping (General, Section, Workflow, Version, Export) |
| Plan Version | Which version of the plan was affected |
| Details | Additional context specific to the action |
Action details vary by type. For example:
- A section updated entry shows the section name and completion percentage change
- An approved entry shows the approver's notes
- A rejected entry shows the rejection reason
- A published entry shows which previous plan was archived
- An exported PDF entry shows the export format
Viewing Audit Logs
You can access audit logs in two ways:
Company-Wide View
Open the Audit Trail tab in the HazCom Plan Hub. This shows all plan activity across the current site in reverse chronological order.
Plan-Specific View
Open any individual plan and scroll to the audit log section. This shows only the actions related to that specific plan.
Filtering
The audit trail supports several filters to help you find specific entries:
| Filter | Options |
|---|---|
| Action | Created, Updated, Section Updated, Submitted, Approved, Rejected, Published, Archived, Exported PDF, Exported DOCX |
| Category | General, Section, Workflow, Export, Version |
| Date Range | From date and To date (company-wide view only) |
Active filters are indicated by a highlighted filter button. Click Clear all filters to reset.
CSV Export
From the company-wide audit log view, click Export CSV to download the complete audit trail as a spreadsheet. The export respects your current date range filters, so you can export just the entries you need.
The CSV file is suitable for:
- Attaching to compliance files
- Providing to OSHA inspectors during an audit
- Sharing with legal counsel
- Internal compliance reviews
- Annual program review documentation
Why the Audit Trail Matters
During an OSHA inspection, you may be asked to demonstrate that your HazCom program is actively maintained. The audit trail answers common inspector questions:
| Inspector Question | What the Audit Trail Shows |
|---|---|
| "When was this plan last reviewed?" | The date and time of the most recent edit or version creation |
| "Who approved this version?" | The approver's name and any notes they added |
| "Has the plan been updated since you added those new chemicals?" | Version creation entries showing when updates were made |
| "How long has this plan been in effect?" | The publication date and version history |
| "Who is responsible for maintaining this plan?" | User attribution on every edit and update |
| "Can you show me the previous version?" | Archived versions are preserved with full history |
Log Entry Display
Each entry in the audit trail displays:
- An icon indicating the action type (color-coded by category)
- The action description in plain language
- A category badge (General, Section, Workflow, Export, Version)
- The version number of the affected plan
- The date and time of the action
- The user name who performed it
- Detail chips showing additional context (section name, status changes, notes)
Entries are paginated for performance. Use the Previous and Next buttons to navigate through large audit logs.
